The MVR will be produced by Stoltz and Company, LTD on behalf of West Star Transportation, Inc. The report will be used to assess you insurability as defined by our insurance carrier. I hereby authorize West Star Transportation Inc. and/or Stoltz and Company, Ltd to produce a Motor Vehicle Report as deemed appropriate to evaluate my insurability for purposes concerning the driving and operation of company vehicles.
